Obtain health history:

Assess urinary system:

Abnormal findings:

• Menstrual history • Obstetrical history • Menopause • Self-care behaviours • Urinary symptoms • Vaginal discharge • Past history • Sexual activity • Contraceptive use • STI contact

• Areas over kidneys and bladder • Kidneys and bladder for tenderness or pain

Assess reproductive system:

• External genitalia (maturity, pain) • Internal genitalia • Vaginal wall (color, texture and integrity) • Cervix (color, position, size, shape, mucosal integrity and discharge)

Urinary – polyuria, hematuria, urinary frequency, urinary urgency, urinary hesitance, nocturia, urinary incontinence and dysuria Reproductive – genital lesions, vaginal discharge, cervical polyps, vaginal and uterine prolapse, rectocele, dysmenorrhea and amenorrhea
